User Agreement (Terms of Service)
E-ChroniBook (Pvt) Ltd · Last updated 8 October 2026
1. Agreement
This User Agreement is between you and E-ChroniBook (Pvt) Ltd (“ChroniBook”) and governs your use of the ChroniBook electronic medical record platform (the “Service”). By signing in, or by accepting these terms during setup, you agree to them. Your employing facility or the Ministry of Health and Child Care (MoHCC) sponsors your account; their agreements with us may add obligations that also bind you.
2. Accounts and credentials
- Accounts are issued by authorised administrators of your facility or ministry office. Do not share, lend, or reuse credentials.
- You are responsible for all actions performed with your credentials. Report suspected compromise immediately.
- We may suspend or deactivate accounts on your organisation’s instruction, on security grounds, or on breach of this agreement.
3. Permitted use
The Service exists for the delivery and administration of health care and for authorised health-system administration. You agree to use it only for lawful purposes within your assigned role and scope, and to access patient information only where your role, your relationship to the patient’s care, or the law authorises it. You must not attempt to access data outside your assigned province or institution, probe or test the system’s security without written authorisation, interfere with the Service, or extract data for unauthorised purposes.
4. Clinical responsibility
ChroniBook is a record-keeping and workflow tool. It supports clinical decisions; it does not make them. All clinical judgements, prescriptions, and actions remain the professional responsibility of the treating clinician under the laws and professional rules of Zimbabwe. Always verify critical information (allergies, doses, results) before acting, and follow your facility’s clinical governance procedures.
5. Data you enter
Clinical and administrative records you enter become part of the patient’s medical record. Enter information accurately, attribute it correctly, and use the system’s correction and amendment flows rather than attempting to alter records covertly. All changes are audited.
6. Availability
We aim for high availability but do not guarantee uninterrupted service. Facilities must keep contingency procedures (paper records, offline queues) for outages. We are not liable for losses arising from unavailability, except to the extent such liability cannot be excluded by law.
7. Intellectual property
The Service — software, design, and documentation — is owned by E-ChroniBook (Pvt) Ltd and its licensors. You receive a non-exclusive, non-transferable right to use the Service for its intended purpose while your account is active. Data in the system belongs to the patients and the health system, not to you or us to dispose of freely.
8. Liability
To the maximum extent permitted by law, neither party is liable to the other for indirect or consequential losses. Nothing in this agreement limits liability that cannot be limited by law, and nothing in it affects patients’ rights or clinicians’ professional obligations.
9. Termination
Your access ends when your organisation deactivates your account, or when we suspend it for security or breach reasons. On termination your pending sessions are revoked. Provisions that by their nature should survive (records integrity, confidentiality, liability) do survive.
10. Changes and governing law
We may update this agreement; material changes bump the version above and you will be asked to accept the new version at sign-in. This agreement is governed by the laws of Zimbabwe, and disputes fall under the jurisdiction of Zimbabwean courts.
